The Truth About High Heels: What They’re Doing to Your Feet

By Dr. Neda Arjomandi, Board-Certified Podiatrist – Laguna Woods Podiatry Group

High heels are synonymous with fashion and elegance. For many, they represent confidence, professionalism, or a special occasion staple. But at what cost? At Laguna Woods Podiatry Group, I frequently treat patients suffering from the long-term effects of wearing high heels. As a board-certified podiatrist, I believe it's important to educate the public on the health implications of high heel use—and how to balance style with foot wellness.

👠 How High Heels Impact Foot Health

When you wear high heels, your body's natural alignment is altered. Your center of gravity shifts forward, forcing your toes into the narrow front of the shoe and increasing the load on the balls of your feet. This seemingly small adjustment in posture can cause a chain reaction of issues from your toes all the way up to your spine.

Here are some common foot problems caused or worsened by frequent high heel use:

1. Bunions and Hammertoes

High heels, especially those with pointed toes, force your toes into cramped, unnatural positions. Over time, this can cause or exacerbate bunions, hammertoes, and other toe deformities. These conditions are often progressive and may eventually require surgical correction.

2. Metatarsalgia

Metatarsalgia refers to pain and inflammation in the ball of the foot, where most of your body weight rests when you're in heels. Thin soles and lack of cushioning make this worse, leading to sharp or burning pain that can interfere with daily activities.

3. Achilles Tendon Tightness and Heel Pain

High heels shorten the Achilles tendon and calf muscles over time. This can lead to tightness, reduced ankle mobility, and a higher risk of Achilles tendonitis or even tendon rupture. Some people also develop plantar fasciitis, a painful condition that affects the heel and arch.

4. Morton’s Neuroma

Tight, high-heeled shoes can compress the nerves between the toes, leading to Morton's neuroma. This nerve irritation causes pain, numbness, and a sensation like stepping on a pebble.

5. Postural and Musculoskeletal Problems

The unnatural alignment caused by high heels doesn’t just affect your feet. It can contribute to knee, hip, and lower back pain, especially when worn for extended periods. The spine’s curvature may change over time, placing extra stress on muscles and joints.

🧵 Tips for Wearing High Heels Safely

If you're not ready to part with your favorite pair of heels, here are a few strategies to help reduce your risk of long-term damage:

Limit heel height: Stick to heels that are 2 inches or lower for regular wear. Save higher heels for special occasions.

Choose supportive styles: Look for heels with wider toe boxes, cushioned insoles, and thicker heels for more stability.

Alternate footwear: Give your feet a break by wearing supportive shoes throughout the day and switching to heels only when needed.

Stretch regularly: Incorporate calf and foot stretches to keep muscles flexible and prevent shortening of the Achilles tendon.

Use orthotics or inserts: Custom orthotics or over-the-counter inserts can provide added support and reduce pressure on key areas.

👩‍⚕️ When to See a Podiatrist

If you experience persistent pain, numbness, swelling, or visible deformities, it’s time to consult a podiatrist. Many conditions related to high heel use can be managed conservatively when caught early. Ignoring symptoms, however, can lead to chronic issues that are harder to treat.

As a board-certified podiatrist, I’m dedicated to helping patients find relief while supporting their lifestyle choices. Whether you're managing an existing condition or seeking advice on preventive care, our team at Laguna Woods Podiatry Group is here to help.
